Who is a Canidate?

Position: Vision Health Coordinator
Location: Franklin Square, NY
Duties:
Assist patients in accessing vision care services, including providing general support under the direction of senior staff.
Provide patients with general education on eye health topics, common vision concerns, and available treatment options to support informed decision-making.
Support care coordination by communicating with optometrists, ophthalmologists, and other healthcare staff to help implement patient care plans.
Refer patients, as appropriate, to external resources such as vision support programs, eyewear assistance programs, or community health education services.
Maintain basic records related to patient engagement and report updates or concerns regarding care plans to supervising personnel.
Identify and report common barriers to care to assist the team in improving patient access and service delivery.
Minimum Requirements:
Bachelor’s degree in Public Health, Health and Society, Health Sciences, or a closely related field.
